Study Title: Assessment of Diet in relation to Public Speaking Anxiety


What is the purpose of this research study?

The main purpose of this study is to assess the relation between diet and anxiety associated with public speaking.

What should I know about the survey?

Whether or not you take part is up to you.

• Your participation is completely voluntary.

• You can choose not to take part.

• You can agree to take part and later change your mind.

• Your decision will not be held against you.

• Your refusal to participate will not result in any consequences or any loss of benefits that you are otherwise entitled to receive.

• You can ask all the questions you want before you decide

How long will the survey last and what will I need to do?

The survey will take 15-20 minutes to complete. You will be asked to fill out the survey online and provide the following information:

• A self-assessment about your public speaking anxiety level

• A self- administered dietary recall

• Your general background information (e.g., demographics, lifestyle)

Is there any way taking this survey could be bad for me?

We believe the risks from participating in this study are minimal. Potential risks could be that you may feel uncomfortable about answering certain questions in the questionnaires. You may choose not to participate. If you choose to participate now, you will retain your right to withdraw from the study later.

Will being in this survey help me in any way?

We cannot promise any benefits to you or others from taking part in this study.

If I take part in this survey, how will my privacy be protected? What happens to the information you collect?

Your participation will be completely anonymous. Your information or the data collected as part of this research will be retained for study record keeping purpose per institutional policy.
